Listed below is information requested by the carrier. While you may be asked to complete a specific application or form, the information requested will typically be the same.
- Most recent experience Mod Worksheet.
- Payroll data for the most recent five years.
- Payroll expected for the upcoming year by class code.
- Workers' compensation premiums for the most recent
five years.
- Audited financial data for past three years.
- Information about your loss control program.
- Copies of actual workers' compensation claim loss history showing both the dollar amount of the claims and the number of claims value dated within past three months.
- Information on the use of any watercraft or any owned, leased, or chartered aircraft.
