We at the Georgia Center for Nonprofits (GCN) recognize the importance of having a good, affordable healthcare insurance program for GCN members.
GCN’s Preferred Healthcare Provider
After recently conducting a thorough market analysis of providers and carefully reviewing all proposals, we selected Aetna as our preferred healthcare partner.
Aetna is one of the nation's leading diversified healthcare-benefits companies. They offer a broad range of traditional and consumer-directed health insurance products and related services, including medical, pharmacy, dental, behavioral health, group life, long-term care, disability plans, and medical management capabilities.
Healthcare Plans
We are very pleased to be able to offer a variety of healthcare plans that meet the diverse needs of our members. GCN will be offering its members the choice of eight healthcare plans, including regional ones.
These plans are designed for groups with two or more employees. Nonprofits with one employee may enroll under one of Aetna’s individual plans.
Small Business Healthcare Insurance
Plan Types
HMO (Health Maintenance Organization)
This is a "network only" plan. If you elect an HMO, you have to see an in-network doctor. If you go out of the network you have no benefit. All of our HMO plans are Open Access, meaning that you do not have to select a primary care physician and you do not need to get a referral to see a specialist.
POS (Point of Service)
This is an HMO plan that provides an "out-of-network" benefit. The plans provide an incentive for you to stay in network by giving substantially stronger benefits if you do. If you do want to go to a doctor who’s not in your network you’ll still have some benefits when you go to that doctor. With the changes over the years, Aetna’s POS plans tend to look and act like PPO plans.
MC (Managed Choice)
Managed Choice plans are just like the POS plans except they utilize a network--contracted by Aetna--for outlying areas of Georgia. For example, Macon, Savannah, and Augusta all have MC plans available but do not have access to the POS network.
PPO (Preferred Provider Organization)
Aetna’s PPO plan looks similar to the MC and POS plans listed above. But with the PPO plan you can access a network that extends into most of the smaller cities and rural communities in Georgia.

Optional Coverage Plans: Vision, Dental, and Term Life
Vision, dental, and term-life plans are optional and not included in the Aetna plans. These optional plans are covered by Century Healthcare. This coverage is available for both individuals and groups.

How to Get a Quote for Health Insurance
Please follow these steps to request to quote for Aetna healthcare plans:
1. Please have all of your employees complete the Group Employee Application below.
Groups 2 -- 9 employees: Please complete all of this form except Section A
Groups 10 -- 50 employees: Do not need to complete Section I
2. Please complete the Group Employer Application below.
Please be sure to list the name of your organization as the name of your group and then “GCN.”
3. Please fax these forms to 404-521-0487

How to Get a Quote for Optional Insurance Coverage: Vision, Dental and Term Life
Please follow these steps to request a quote for optional insurance coverage through Century Healthcare:
1. Review the documents that appear above. These documents include rate information.
2. Complete the forms.

INDIVIDUAL APPLICATION
Although previously one-person employers were considered a "group" under the old Blue Cross Blue Shield plans, the state of Georgia now prohibits this practice. The state of Georgia now defines a group as a minimum of two employees. All one-person employers must enroll under the Aetna Individual Plans.
